Mechanism of Action

Sertraline is a selective serotonin reuptake inhibitor that potentiates serotonergic activity in the central nervous system by blocking the reuptake of serotonin at the presynaptic neuronal membrane, thereby increasing the availability of this neurotransmitter in the synaptic cleft.

Sertraline binds with high affinity to the serotonin transporter (SERT) on presynaptic nerve terminals. By occupying the transporter binding site, sertraline prevents the reabsorption of serotonin back into the presynaptic neuron following its release into the synapse. This inhibition of serotonin reuptake leads to an accumulation of serotonin in the synaptic space, allowing sustained activation of both postsynaptic and presynaptic serotonin receptors (including 5-HT₁A, 5-HT₁B, 5-HT₂A, and 5-HT₃ subtypes). The enhanced serotonergic neurotransmission gradually normalises mood, anxiety, and compulsive behaviour patterns over several weeks of continuous therapy.
