Mechanism of Action
Sildenafil is a potent and selective inhibitor of phosphodiesterase-5 (PDE5), the predominant phosphodiesterase isoenzyme responsible for the hydrolytic degradation of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum smooth muscle and pulmonary vascular smooth muscle.
The mechanism is initiated by sexual stimulation or nitric oxide signalling, which triggers the release of nitric oxide from endothelial cells lining the corpus cavernosum and pulmonary vasculature. Nitric oxide diffuses into adjacent smooth muscle cells and activates soluble guanylate cyclase, the enzyme that catalyses the conversion of guanosine triphosphate to cGMP. The accumulating cGMP activates protein kinase G, which phosphorylates multiple downstream targets including calcium-sensitive potassium channels and phospholamban, resulting in reduced intracellular calcium concentration. This calcium depletion causes relaxation of the corpus cavernosum trabecular smooth muscle and dilatation of the helicine arteries, allowing blood to flow into the lacunar spaces and produce penile erection. In the pulmonary circulation, the same pathway reduces pulmonary vascular resistance and right ventricular afterload.
Sildenafil is approximately 4,000-fold more selective for PDE5 than PDE3 and PDE4, but shows only 10-fold selectivity over PDE6 found in retinal photoreceptors, which accounts for the mild and transient visual disturbances reported at higher doses. The therapeutic effect lasts approximately four to six hours, with peak plasma concentration reached 30 to 120 minutes after oral administration.

- High-Altitude Pulmonary Oedema (HAPE): Sildenafil has been used prophylactically and therapeutically to prevent and treat HAPE by reducing pulmonary artery pressure without compromising systemic oxygenation.
