Mechanism of Action

Sodium Picosulfate works as a stimulant laxative through a two-step mechanism. It is a prodrug that has no significant direct effect on the intestine itself. After oral administration, the molecule passes through the stomach and small intestine without being absorbed. Upon reaching the colon, gut bacteria hydrolyse sodium picosulfate to its active metabolite, bis-(p-hydroxy-phenyl)-pyridyl-2-methane (BHPM), also known as desacetyl bisacodyl. This active metabolite acts directly on the colonic mucosa, where it stimulates sensory nerve endings in the enteric nervous system. This stimulation increases the frequency and force of peristaltic contractions throughout the large bowel, propelling faecal matter toward the rectum. At the same time, BHPM reduces the absorption of water and electrolytes from the colonic lumen, which softens the stool and increases bulk. The combined effect of enhanced motility and reduced water absorption produces a laxative action within six to twelve hours. Unlike bulk-forming laxatives that require adequate fluid intake to swell, sodium picosulfate works independently of dietary fibre and is suitable for patients who need reliable bowel evacuation before diagnostic procedures or surgery.

Therapeutic Uses & Indications

Therapeutic Uses

Sodium Picosulfate is prescribed for the treatment of:

  • Constipation: Provides effective short-term relief of constipation by stimulating bowel movement within 6–12 hours of administration.
  • Bowel Cleansing for Colonoscopy: Used in combination with magnesium citrate for colonic preparation before colonoscopy, ensuring clear visualization of the colonic mucosa.
  • Pre-Surgical Bowel Evacuation: Administered before abdominal or colorectal surgery to empty the bowel and reduce the risk of contamination.
  • Radiological Investigations: Used to clear the bowel before barium enema or other imaging studies of the gastrointestinal tract.

Why Manufacturers Choose Sodium Picosulfate

Sodium Picosulfate is one of the most widely manufactured stimulant laxatives in India due to its predictable efficacy, excellent safety profile, and multiple dosage form options. It is available as tablets, oral drops, and oral solution, allowing manufacturers to cater to both paediatric and adult populations. The molecule is highly cost-effective to produce, with well-established synthetic routes and readily available raw materials. Indian manufacturers produce sodium picosulfate for both the domestic market and export to over 50 countries, including regulated markets such as Europe and the Middle East. Its inclusion in the WHO Essential Medicines List further reinforces its的地位 as a staple in gastrointestinal pharmacotherapy. The combination product with magnesium citrate is particularly popular among PCD franchise operators in India, driving consistent demand throughout the year.

This makes Sodium Picosulfate one of the most sought-after molecules for third party manufacturing in India, with consistent demand from PCD franchise operators and export-oriented companies.
