Mechanism of Action

Tacrolimus works by inhibiting T-lymphocyte activation through a targeted intracellular mechanism. Once it penetrates the cell membrane, Tacrolimus binds to a specific intracellular receptor protein called FK506-binding protein 12 (FKBP-12). The resulting Tacrolimus-FKBP-12 complex then binds to and inhibits calcineurin, a calcium-dependent phosphatase enzyme. By blocking calcineurin activity, the complex prevents the dephosphorylation and subsequent nuclear translocation of the nuclear factor of activated T-cells (NFAT). This inhibits the transcription of genes encoding inflammatory cytokines, such as interleukin-2, interferon-gamma, and tumor necrosis factor-alpha.

Unlike topical corticosteroids, Tacrolimus does not inhibit collagen synthesis and does not cause skin thinning, skin atrophy, or telangiectasia.
