Thiopentone Sodium is an ultra-short-acting barbiturate that produces rapid loss of consciousness by potentiating GABA-A receptor activity in the brain. It binds to the barbiturate binding site on the GABA-A receptor-chloride ionophore complex, increasing the duration of chloride channel opening in response to GABA. This enhances inhibitory neurotransmission throughout the central nervous system, particularly in the reticular activating system, thalamus, and cerebral cortex. The enhanced GABAergic inhibition leads to decreased neuronal excitability, reduced synaptic transmission, and ultimately general anesthesia. The onset of action is extremely rapid (30-45 seconds after intravenous injection) due to the drug's high lipid solubility, which allows rapid penetration of the blood-brain barrier. Consciousness returns in 5-10 minutes as the drug redistributes from the brain to peripheral tissues such as skeletal muscle and adipose tissue.
