Mechanism of Action
Timolol Maleate is a non-selective beta-adrenergic receptor blocking agent that reduces intraocular pressure by decreasing the production of aqueous humour in the eye. It works by competitively blocking both beta-1 and beta-2 adrenergic receptors in the ciliary epithelium, the tissue responsible for producing the fluid that fills the front of the eye. When these receptors are blocked, cyclic AMP levels within the ciliary body decrease, which slows the active secretion of aqueous humour. This reduction in fluid production lowers the pressure inside the eye, preventing damage to the optic nerve. Timolol does not have significant intrinsic sympathomimetic activity, direct myocardial depressant activity, or local anaesthetic membrane-stabilising activity, making it a clean beta-blocker for ophthalmic use. The drug is the levo-isomer of timolol, which is more potent than the racemic mixture. Despite being applied topically as eye drops, a portion is absorbed systemically through the nasolacrimal duct, which can produce systemic beta-blocking effects.
