Mechanism of Action

Tobramycin is a broad-spectrum aminoglycoside antibiotic that exerts its bactericidal effect by irreversibly binding to the 30S ribosomal subunit of susceptible bacterial cells, specifically targeting the 16S ribosomal RNA (rRNA) component within the A-site of the 30S ribosome. By binding to this decoding region, tobramycin interferes with the initiation step of translation, causing misreading of mRNA codons and leading to the incorporation of incorrect amino acids into growing polypeptide chains. This disruption of protein synthesis results in the accumulation of aberrant, non-functional proteins within the bacterial cell, which damages the cell membrane and leads to rapid bacterial cell death.

Tobramycin is particularly active against aerobic Gram-negative bacteria, with its most notable clinical activity against Pseudomonas aeruginosa. Its spectrum also includes Escherichia coli, Klebsiella pneumoniae, Enterobacter species, Proteus species, and Serratia marcescens. The drug shows synergistic activity when combined with beta-lactam antibiotics against certain Gram-negative organisms. Tobramycin is poorly absorbed from the gastrointestinal tract, and for systemic infections, it is administered parenterally.
