Mechanism of Action

Torsemide is a loop diuretic that produces its pharmacological effect by reversibly inhibiting the sodium-potassium-chloride cotransporter (NKCC2) located on the luminal membrane of the thick ascending limb of the loop of Henle. By binding to the chloride transport site of this carrier protein, torsemide prevents the reabsorption of sodium, chloride, and potassium ions from the tubular lumen back into the medullary interstitium. The resulting increase in osmotically active solutes within the tubular lumen draws water along by osmotic gradient, producing a pronounced increase in urine output.

The natriuretic effect of torsemide is dose-dependent and can result in the excretion of up to 20 to 25 percent of filtered sodium load. Unlike thiazide diuretics that act on the distal convoluted tubule, loop diuretics such as torsemide do not lose efficacy in patients with reduced kidney function because the thick ascending limb continues to reabsorb sodium even in chronic kidney disease. Torsemide also causes venodilation and reduces preload in patients with heart failure through a prostaglandin-mediated mechanism that precedes its diuretic effect.
