Mechanism of Action

Verapamil Hydrochloride is a phenylalkylamine calcium channel blocker that selectively inhibits the transmembrane influx of calcium ions through L-type voltage-dependent calcium channels in cardiac and vascular smooth muscle cells.

By blocking these slow calcium channels, verapamil reduces intracellular calcium availability, which is essential for excitation-contraction coupling. In vascular smooth muscle, this leads to coronary and peripheral arterial vasodilation, decreasing systemic vascular resistance and lowering blood pressure. Unlike dihydropyridine calcium channel blockers, verapamil does not cause reflex tachycardia because it also acts directly on the sinoatrial and atrioventricular nodes.

In cardiac tissue, verapamil prolongs the effective refractory period within the AV node and slows AV conduction in a rate-related manner. This negative dromotropic effect makes it highly effective for controlling ventricular rate in supraventricular tachyarrhythmias.
