Mechanism of Action

Vincristine is a vinca alkaloid that inhibits cell division by binding to tubulin and preventing microtubule polymerization. Microtubules are essential for forming the mitotic spindle, the cellular machinery that separates chromosomes during cell division. By binding to tubulin dimers, vincristine blocks their assembly into microtubules, causing the dissolution of the mitotic spindle. Dividing cells are arrested in metaphase and subsequently undergo apoptosis. Vincristine is cell cycle-specific, acting primarily during the M-phase when the mitotic spindle is being formed. It is extracted from Catharanthus roseus and is structurally similar to vinblastine but with a formyl group substitution that gives it distinct pharmacological properties. The most notable difference is vincristine's pronounced neurotoxicity, which is dose-limiting, and its minimal bone marrow suppression at standard doses. This unique toxicity profile makes vincristine an ideal component of combination chemotherapy regimens, where it can be combined with full doses of other myelosuppressive agents without additive bone marrow toxicity. Vincristine also interferes with other cellular processes including nucleic acid and lipid biosynthesis at higher concentrations, contributing to its antineoplastic activity.

Therapeutic Uses & Indications

Therapeutic Uses

Vincristine Sulphate is prescribed for the treatment of:

  • Acute Lymphoblastic Leukemia: A cornerstone drug in ALL induction, consolidation, and maintenance therapy, especially in pediatric patients where it has dramatically improved survival rates.
  • Hodgkin and Non-Hodgkin Lymphoma: Used in standard regimens including CHOP, MOPP, and COPP protocols.
  • Wilm Tumor: An essential component of the standard treatment protocol for this childhood kidney cancer.
  • Neuroblastoma: Used in combination therapy for high-risk neuroblastoma in children.
  • Rhabdomyosarcoma: A standard component of chemotherapy for this soft tissue sarcoma in both children and adults.
  • Ewing Sarcoma: Included in multidrug regimens for this bone and soft tissue cancer.
  • Idiopathic Thrombocytopenic Purpura: Used off-label in refractory ITP cases unresponsive to splenectomy and steroids.

Why Manufacturers Choose Vincristine Sulphate

Vincristine Sulphate is classified as an essential medicine by the WHO and plays an irreplaceable role in pediatric oncology, particularly in ALL treatment where it has contributed to cure rates exceeding 80% in children. This essential status ensures consistent global demand. Indian manufacturers can produce both the liquid injection and lyophilized powder forms, with the 1mg/ml liquid injection being the most commonly used formulation in hospitals. Vincristine is one of the highest-volume oncology items in the Indian PCD franchise market and has strong export demand across emerging and regulated markets. WHO-GMP certified facilities with cytotoxic drug handling capabilities are necessary for manufacturing, ensuring quality standards that command premium pricing in regulated markets.
