Mechanism of Action
Voriconazole is a broad-spectrum triazole antifungal agent that exerts its activity by inhibiting fungal cytochrome P450-dependent 14-alpha-lanosterol demethylation, a critical step in the biosynthesis of ergosterol. Ergosterol is an essential component of the fungal cell membrane, playing a role analogous to cholesterol in mammalian cells by maintaining membrane integrity, fluidity, and proper function of membrane-bound proteins. Voriconazole specifically binds to and inhibits the fungal enzyme lanosterol 14-alpha-demethylase, which is encoded by the CYP51 gene. This inhibition prevents the conversion of lanosterol to ergosterol, causing a progressive depletion of ergosterol in the fungal cell membrane. At the same time, the accumulation of toxic 14-alpha-methylated sterol precursors disrupts membrane structure and function. The resulting damage to the fungal cell membrane leads to increased membrane permeability, impaired nutrient transport, disrupted cell signaling, and ultimately fungal cell death. Voriconazole exhibits fungicidal activity against Aspergillus species and fungistatic activity against Candida species. Its spectrum of activity includes Aspergillus fumigatus and other Aspergillus species, Candida albicans and non-albicans Candida species, as well as emerging and difficult-to-treat pathogens including Scedosporium apiospermum and Fusarium species. Voriconazole is metabolized by hepatic cytochrome P450 enzymes, primarily CYP2C19, with contributions from CYP2C9 and CYP3A4. It also acts as a strong inhibitor of CYP3A4 and an inhibitor of CYP2C9 and CYP2C19, which gives it a significant potential for drug-drug interactions that must be carefully managed in clinical practice.
