Xylometazoline hydrochloride is a sympathomimetic imidazoline derivative that acts as a direct-acting alpha-adrenergic receptor agonist. When applied topically to the nasal mucosa, xylometazoline binds to and activates alpha-1 and alpha-2 adrenergic receptors located on the smooth muscle cells of nasal blood vessels. The primary alpha-adrenergic receptor subtypes present in human nasal mucosa include alpha-2A, alpha-1A, and alpha-2B receptors. Activation of these receptors triggers a G-protein-coupled signaling cascade that leads to vasoconstriction of the dilated arterioles and small arteries within the nasal mucosa. This vasoconstriction reduces blood flow to the engorged and inflamed nasal tissues, thereby decreasing mucosal swelling, edema, and congestion. The decongestant effect begins within minutes of administration and lasts for up to 10 hours, providing sustained relief from nasal obstruction.
