Mechanism of Action

Zoledronic acid is a third-generation nitrogen-containing bisphosphonate that works primarily by inhibiting osteoclast-mediated bone resorption. When administered intravenously, it rapidly binds to hydroxyapatite crystals in the bone mineral, especially at sites of high bone turnover such as areas affected by osteoporosis, metastatic lesions, or Paget's disease. Once localized, zoledronic acid is released during the bone resorption process and taken up by osteoclasts — the cells responsible for breaking down bone tissue. Inside the osteoclast, it inhibits farnesyl pyrophosphate synthase, a key enzyme in the mevalonate pathway. This inhibition prevents the prenylation of small GTPase proteins that are essential for osteoclast function and survival. As a result, osteoclast activity is suppressed, and the cells undergo programmed cell death or apoptosis. This leads to a significant reduction in the rate of bone turnover. The high binding affinity of zoledronic acid to bone mineral gives it a remarkably long duration of action, allowing for once-yearly intravenous dosing in osteoporosis.
