Product Overview
METCIAN is an essential biguanide oral hypoglycemic agent formulated to manage type 2 diabetes mellitus. It provides excellent glycemic control by improving insulin sensitivity and optimizing glucose utilization across peripheral muscle tissues.
Indications
- Type 2 Diabetes Mellitus: Used as monotherapy or combined with other antidiabetic agents to control blood glucose levels.
- Insulin Resistance: Assists in managing metabolic syndromes and pre-diabetic clinical states.
- Polycystic Ovary Syndrome (PCOS): Off-label management to regulate menstrual cycles and lower systemic insulin resistance.
Dosage & Administration
The standard adult starting dose is one 500mg tablet taken once or twice daily during or immediately after major meals to reduce gastrointestinal side effects. Dosages must be individualized by a physician.
Adverse Effects
Common adverse effects include transient gastrointestinal symptoms such as nausea, vomiting, abdominal bloating, diarrhea, a metallic taste in the mouth, and localized vitamin B12 malabsorption over long-term use.

Contraindications
Contraindicated in patients with severe renal impairment (eGFR below 30 mL/min/1.73m²), acute or chronic metabolic acidosis, diabetic ketoacidosis, and clinical conditions predisposing to tissue hypoxia.
Warnings & Precautions
Lactic acidosis is a rare but severe metabolic complication that can occur due to drug accumulation in patients with significant renal dysfunction. Discontinue therapy prior to iodinated contrast imaging studies.
Drug Interactions
Cationic drugs like cimetidine and dolutegravir may reduce metformin clearance, increasing its blood concentration. Furosemide and alcohol can potentiate the risk of lactic acidosis.
