TELNASTAR-M is a modern dual-action anti-diabetic bilayered tablet formulated to provide glycemic regulation in Type 2 Diabetes Mellitus. The combination couples a dipeptidyl peptidase-4 inhibitor with a sustained-release biguanide to manage fasting and postprandial blood glucose levels effectively.
Indications
This tablet is indicated for:
- Type 2 Diabetes Mellitus: Management of glycemic control in adults when metformin monotherapy or teneligliptin alone provides insufficient metabolic regulation.
Dosage & Administration
Adults: One tablet daily with the evening meal, or as advised by an endocrinologist. Swallow the tablet whole; do not crush, split, or chew.
Adverse Effects
- Hypoglycemia (especially when combined with sulfonylureas)
- Gastrointestinal symptoms like diarrhea, nausea, and abdominal flatulence

Contraindications
Contraindicated in patients with severe renal impairment (eGFR less than 30 mL/min), acute or chronic metabolic acidosis (including diabetic ketoacidosis), or hypersensitivity to the components.
Warnings & Precautions
Metformin accumulation can rarely lead to lactic acidosis, a serious metabolic complication; monitor renal profiles routinely. Discontinue therapy during acute conditions associated with dehydration or hypoxemia.
Drug Interactions
Cationic drugs like cimetidine or dolutegravir may increase plasma levels of metformin. Concurrent administration with glucocorticoids or diuretics can elevate blood glucose levels, requiring dose adjustments.
