TOTAAM-4.5 is a gold-standard, broad-spectrum injectable antibacterial combination designed to treat severe, complicated systemic infections. By pairing an extended-spectrum penicillin with a potent beta-lactamase inhibitor, it provides robust clinical efficacy against highly resistant Gram-positive, Gram-negative, and anaerobic pathogens.
Indications
This intravenous antibiotic is clinically indicated for the management of:
- Severe Nosocomial Pneumonia: Targets resistant respiratory pathogens in hospital settings.
- Complicated Intra-abdominal Infections: Treats deep intra-abdominal abscesses, peritonitis, and biliary tract infections.
- Complicated Skin and Soft Tissue Infections: Eradicates deep tissue infections, including diabetic foot ulcers and ischemic gangrene.
- Gynecological Infections: Resolves severe postpartum endometritis and pelvic inflammatory disease.
Dosage & Administration
To be reconstituted and administered strictly via slow intravenous infusion over a period of 30 minutes. The typical adult dose ranges from 3.375 g to 4.5 g every six hours, depending on the site and clinical severity of the infection. Dose adjustments are required in patients with compromised renal function.
Adverse Effects
Commonly reported adverse clinical reactions include:
- Diarrhea, nausea, and vomiting
- Localized pain, phlebitis, or swelling at the intravenous injection site
- Transient skin rashes or hypersensitivity pruritus

Contraindications
Contraindicated in individuals with a history of severe, immediate hypersensitivity reactions (such as anaphylaxis) to penicillins, cephalosporins, or beta-lactamase inhibitors.
Warnings & Precautions
Serious and occasionally fatal hypersensitivity reactions can occur. Before starting therapy, conduct a thorough medical inquiry regarding prior drug allergies. Monitor renal, hepatic, and hematologic functions during prolonged courses of treatment. Pseudomembranous colitis should be suspected if severe, persistent diarrhea develops.
Drug Interactions
Concomitant use with aminoglycosides may result in mutual inactivation; therefore, they should be administered separately. It competes for renal excretion with methotrexate, potentially increasing methotrexate toxicity risks. Co-administration with oral anticoagulants or heparin requires frequent monitoring of coagulation profiles.
Storage Instructions
Store the dry sterile powder vials below 25°C. Once reconstituted with a compatible sterile diluent, the solution must be used immediately or stored according to validated hospital refrigeration guidelines.
